Health Services Administration Internship Application
To be eligible for an internship you must:
- Have an overall GPA of at least 2.5
- Be currently admitted to the Bachelor of Health Science program
- Have completed at least 18 hours of BHS courses

To fill out this form you will need:
- Your availability for the semester of the internship you are applying
- List of BHS courses you've completed
- Name and information of two references who are not BHS faculty members
- A short statement (approx. 500 words) about why you are applying for an internship.
Submitting an application does not guarantee internship placement. All applications are reviewed by the BHS faculty for approval.
