Request Declaration of Program (Major)
Note: Until formally admitted into an Allied Health program, you will be listed as a "Pre" major. (For example, Pre-Radiologic Technology or Pre-Diagnostic Medical Sonography)
Students interested in declaring a major in one of the Allied Health programs should do so in consultation with the appropriate faculty member (see list below).
