Institutional Review & Approval Forms
The Office of Sponsored Projects coordinates the review and institutional approval process for all externally funded proposals. To support this process, the following forms are required for all external funding opportunities:
- Intent to Submit Form: Notifies the Office of Sponsored Projects of planned proposal submissions to facilitate institutional coordination.

Compliance & Monitoring Forms
To ensure compliance and protect Washburn, we have established a system of compliance checks and accountability measures.
- Grant Monitoring Review Form: This form facilitates the internal review of post-award reports and other sponsor-required documents to ensure they are complete and appropriately reviewed before submission.
- Conflict of Interest Form: The Conflict-of-Interest Disclosure Form is used to identify, disclose, and manage potential conflicts that could influence sponsored project activities.
